The main thing keeping it from 5 stars is the thickness, or more accurately, the standoff between the face of the controller and the mount. It slightly interferes with the Options and Share buttons, and the touchpad ends up "recessed" inside the mount by about 1/8". That's not a lot in the bigger world, but on something as precisely designed as a controller, it's noticeable. I'm sure this makes the mount much more hardy and less prone to cracking, but it's still distracting.
And although it should be obvious, attaching your phone to your DS4 is going to affect the overall feel and balance. Again, there are bigger problems in the world, and you'd probably get used to it over time, but it's awkward at first.
(It does make using the Fallout 4 Pip-Boy app a ton of fun, though.)
1: Sits close to the options (and share) buttons, and makes them a little harder to press. Also interferes with the docking station, so it has to be pulled all the way off to charge. Detaches very easily, though
2: Grips aren't thick enough to hold a phone with a serious (otterbox) case. I have to take the hard plastic shell off for it to seat right. Less a problem then a suggestion for the next rev, though.
3: Holding the phone up and out like that creates a surprising amount of torque for such a light controller. It completely changes the balance, which means the controller won't sit in your hand anymore, it's trying to roll out. Again, I don't see there being a good fix for this, and you'll probably get used to it pretty quick.
