Measure the circumference of your elbow:
Small: 7" (18cm) to 7" (20cm) - no that isn't a typo, thats what it says on the box. Well it isn't MY typo anyway.
Medium: 8" (20cm) to 9" (23cm)
Large: 9" (23cm) to 10" (25cm)
X-Large 10" (25cm) to 12" (30cm)
For your reference, my elbow measures a shade over 10". I have a bit of muscle on me, but I'm not what you'd consider jacked. They should of course be tight, but the medium, on me, are definitely TOO tight. Nearly impossible to get them all the way on and if I did I think I'd lose circulation. I've ordered the large and am returning the medium, so I'll update this review with performance/quality info once I've gotten to use them.
I am 5'8" and 160lbs, ectomorph body type, lift weights and run regularly. The LARGE size fits perfect on me with 15-3/4 biceps. It fits skin-tight and a medium would definitely be too small.
The sleeve is definitely thick. It fits under a jacket but not under a skin-tight Under Armour type shirt. I just wear it over it, and then put a jacket on. Currently, I use the sleeve to keep my elbow warm as I developed a slight case of tennis elbow from doing heavy decline barbell benchpresses. The sleeve does give a lot of support and I'm satisfied with the product.
I returned mine and bought the ones from McDavid. It's a much better product and a fraction of the cost. No regrets on the exchange.
