First, the power plugs are both connected together; the barrel plug and the screw down posts are electrically connected, so you only use one. You can use one or the other, and whichever you use will inject that voltage into the Ethernet cables. The board has a resister to turn on the LED, which is the only onboard process or logic. Most of the top of the board is a positive plate, and the bottom is mostly negative besides a few traces, which I have outlined in a picture in case it is needed.
Since the top of the board is mostly positive polarity, if you screw in the board to something, be careful that you do not damage the epoxy coating on the board. If the coating is damaged, the screws will also be charged with whatever voltage is being injected. I suggest using fiber insulating washers, just like the ones that come with most computer motherboards, such as these uxcell PC Motherboards Screw Insulating Fiber Washers Red Pack of 100. If you are mounting to a rack or something metal, you are going to want some normal spacers like these Teenitor 120Pcs M3 Nylon Hex Spacers Screw Nut Stand-off Plastic Accessories Assortment White or another plastic spacer to prevent shorting the contacts to whatever you are mounting it to. I put mine on a network rack, so I just drilled and tapped some 6-32 holes and used plastic standoffs and normal 6-32 screws that tend to be all over from building anything 'computery'. But if you need some, you can grab some Estone 100pcs Toothed Hex 6/32 Computer PC Case Hard Drive Motherboard Mounting Screws or find that old computer you are about to toss and remove all the screws from the case.
The pictures show which traces and which screw terminals are positive and negative, with the positive voltage traces in red. Check that the traces run the same way on yours, as there could be different versions of the board.
This injector is awesome. Simple design with very few components. That's exactly what I wanted. The fewer extras or added features the better, that's less things to fail. This thing passes the data lines through and pushes whatever power you connect to it on the PoE pins. You can't beat this, especially for the price. I kind of wish it had a cover or something over the bottom of the board (shocked myself not thinking about it).

Note that this device is not a network switch, so each network port has to be connected to a switch.
